What this application collects
The form collects the applicant’s name, Louisiana city, selected private contact route, age-and-residency confirmation, goal, prior barrier, commitment evidence, training access, schedule information, current coaching status, required acknowledgements, typed signature, and a minimized yes/no/private safety flag.
Do not submit diagnoses, medical records, medication details, payment information, passwords, credentials, government identification, customer lists, or other sensitive documents through this public form or social media.
Why PKFIT uses it
PKFIT uses application data only to administer this campaign: receive and deduplicate applications, verify eligibility, conduct the merit review, contact shortlisted applicants, complete private finalist screening, document decisions, and address campaign questions or disputes.
Application contacts will not be added to a general marketing list, sold, rented, or reused for unrelated outreach. Public-media interest is not collected in this application and is not part of scoring.
Processor and storage path
Netlify processes the web submission on PKFIT’s behalf and stores the response in the account-controlled Netlify Forms environment. Authorized PKFIT operators may export the minimum fields needed for review into a private, access-restricted working file. Netlify’s own infrastructure, security, backup, and subprocessor practices also apply while it holds the submission.
Access and review controls
Access is limited to authorized campaign administrators and reviewers who need it for the purposes above. Reviewers should use candidate IDs and redacted scoring packets where practical. The acquisition scoreboard may contain only aggregate counts or non-reversible IDs, never names, contacts, health flags, selection notes, or consent records.
Health and selected-applicant information
The public safety flag is not a diagnosis and is not scored. Detailed health screening, emergency contact information, clinician clearance, and coaching records are requested only after provisional selection through an approved private onboarding route. Those records are not stored in the public campaign form.
Retention and deletion procedure
PKFIT targets deletion of nonselected application responses and derived review exports within 30 days after the final selection decision or campaign close, whether zero, one, or two seats are filled. An authorized operator deletes the records from Netlify Forms and the restricted local review workspace, records completion in a private deletion log, and verifies that working exports and trash locations no longer contain them.
For selected participants, the minimum information required for onboarding may be transferred into the active client record; the campaign copy is then removed within the same 30-day window. The active client record follows PKFIT’s ordinary coaching-record obligations. Processor backups may persist for a limited period under the processor’s backup cycle even after account-level deletion.
Your choices
You may ask what campaign information PKFIT holds about you or request correction or deletion by emailing the campaign contact. PKFIT will honor the request where feasible and lawful, subject to limited records needed for fraud prevention, dispute handling, or legal obligations.
Public use of a participant’s name, image, measurement, testimonial, or progress material is optional and requires a later separate, specific release. Declining public media has no effect on selection, retention, coaching, or the sponsored fee.
